streamlined application form and the
Adobe Acrobat Reader you can complete
the process at your PC in just a few
minutes. To apply, just do the
following:
(1) Click on
the button below to open the application
form in PDF format. To move around the
form just press your TAB key. Phone
numbers and social security numbers
should be entered numerically, with no
punctuation, while dates must be
formatted as XX/XX/XXXX. Once completed,
just print, sign, and bring to the
credit union office with your picture
ID. If your location or schedule makes
it difficult for you to visit us in
person you can mail the completed
application and a copy of your picture
ID to our Sacramento office.
(2) If you
have difficulty with the above method,
we recommend trying the following steps.
Right click this link, select "Save
Target As" and place it on your Desktop.
You can then open it by double-clicking
the mbrapp.pdf icon on your Desktop.
Once the Acrobat Reader opens up the
document, complete, print, and deliver
the form as directed above.
(3) To
initiate or change your payroll
deduction, complete and return this form.

To help
the government fight the funding of

identifies each person who opens an

you open an account, we will ask for
your name, address, date of birth, and
other information that will allow us to
identify you. We may also ask to see
your driver's license or other
identifying documents.
